Youth Ministry Registration 2022-2023

Registration for Youth Ministry is now open! We have an awesome year ahead of us and I can't wait to share it with your family and your teen! Registration this year is online, making it easier for you to register!

Use the form below to register. Please reach out to the Office of Youth Ministry, if you need assistance registering, need to utilize a payment plan, or would like to pay with cash or check. Please register before the parent meeting!

  By signing this form, I grant permission for my child, and/or the youth for which I have been granted guardianship, to be photographed, recorded and/or videoed. I understand that youth may decline to be photographed at any time. I further grant permission for the resulting photograph, recording and/or video to be edited, published, live streamed, and/or broadcasted for the purposes of promoting Youth Ministry programs at St. Andrew and elsewhere.
- Requirements
  
  The following are the requirements for the Confirmation Program at St. Andrew. The listed requirements are the minimum requirements to be in the program. Additional events or requirements may be advertised at a later date.
  
  Attend the 7 scheduled Youth Ministry sessions following the 12:15 Mass
  
  Attend 3 social events and/or assist with fundraising events
  
  Weekly Mass
  
  Completion of an approved service project as a group or individual
  
  Attendance at Retreat Year I on May 21st and Year II on February 24-25
  
  Confirmation Requirements YEAR II:
  
  Sponsor Verification Form with church seal (due by January 1, 2023)
  
  Confirmation Saint Name Project (due by March 1, 2023)
  
  Candidate & Sponsor Interview with the Office of Youth Ministry (sign up for a slot)
  
  Attendance at the Retreat
- Payment
  
  Registering more than one teen? If you have more than one teen in the Youth Ministry program you are eligible for the family rate which is $100 for each additional teen in the program. If you have more than 1 teen please register the first one using the "Teen Registration" for the first teen and use the "Family Rate" option for subsequent registrations.
  
  Need a payment plan? If you require to make arrangements to pay with a payment plan please use the "Payment Plan" option to pay your deposit and make arrangements with the youth ministry office to set up a payment plan schedule.
